Purpose of collecting data
Used to help create a Short term treatment plan.
Will be destroyed on the request of either the patient on completion of any treatment, or by request of the patient at anytime.
Patients will be given a written copy of each session they attend if requested.
No data will be shared with any third parties and kept safe from the public and destroyed immediately on request by the patient.
